Thresholds Health is a new, independent community health center in Chicago’s Austin community.

A New Chapter in Care at Thresholds Health

Starting August 18, 2025, Thresholds Health will officially transition operations of our clinic at 5801 W. Corcoran Place to PCC Community Wellness Center.

This partnership ensures continued access to high-quality, community-based healthcare for our clients. PCC is a trusted, mission-driven provider with deep roots in the Chicago area.

To prepare for the opening of the new clinic in its current location, the current clinic will be closed from August 18, 2025, through September 3, 2025. During this time, you are welcome to continue your care at any PCC Community Wellness Center’s clinic location. While you may visit any PCC location, the closest location is just four blocks away at 5461 W. Lake Street. The clinic is reachable by phone at 773-378-3347.

Thresholds is one of the oldest and largest providers of mental health and substance use recovery services in Illinois. We’re creating our community health centers with the goal of learning neighbors’ biggest concerns about health care, whether it’s a need for dermatologists or doctors specializing in heart disease or hypertension.

Our mission is to provide access to this care and be a one-stop shop for the most vulnerable people in our community.
